I have similar problems with a similar configuration. I have tried resetting the network on the iPhone. (Settings/General/ Reset)
Sometimes when I simply "wake up" the iPhone, the connection restores.
I find the hotspot feature "spotty."

The wifi on the iPhone powers down after 15 minutes idle in sleep - if no connections are active (tethering) it will power down - otherwise you'd have 6 hour battery life on your iPhone. Since the iPad uses the same rule, once the iPad sleeps, it drops its wifi... and then your iPhone hotspot will power down as well.
If you plug the iPhone into AC power, it won't power down wifi/hotspot, so if you want the hotspot to stay "live" that's the only way to do it.
This is by design. Unless the iPhone was roughly 4x bigger and all battery, there is no way to keep the hotspot powered up all the time on battery and make it thru a day of use
