Recently got a 47" Vizio LCD and hooked it up to U-verse with HDMI. Didn't have an HDTV before so this is the first time I've used HDMI with this receiver.
Shortly after I first had everything hooked up, I would get this error for a split second (video and audio both stop) and then everything would resume normally. This happens once every hour or so...sometimes more often, sometimes less often. The error screen generally goes away so quickly it took me a while before I could even catch the error number, it always corrects itself within a few seconds.
I've done plenty of resets with/without cables attached and tried different HDMI cables. No change.

Is there an update for your TV on the Vizio website?
Could try making sure the TV is fully on before 'turning on' the DVR/STB. Seems like the TV is not communicating fast enough to avoid the error before it self-corrects.
Only other alternative is component cables w/audio cables, picture should be very much the same, but no errors involved.

TDR21 - A couple more suggestions -
Swap the STB with another one if you have more than one.
Return the TV for another one.
