If you ordered a self-install option, you got exactly what you ordered. The technician brought the service to your NID, then disconnected all the lines form teh house so they don't cause interference on your new service.
Pick the wall jack you want to connect your RG (Residential Gateway) into, and wire that jack only to the proper connections inside your NID. That will feed the signal from your NID to your RG, and give you internet service. Next, plug your phoine into the port on the back of the RG and you should be ready to go.
There are things you can do to feed the dial tone back to the rest of the jacks in your house, but let's first get your service working...
