U-verse phone service added without my authorizat ion

On 05/14/2012 I placed an online order to get my TV service reinstalled. Since I already had the equipment, I called tech support so the service could be restored the same day.
Yesterday, I went online to check listings and I almost fell when I saw a bill for $353.45!!!
After further review, I noticed that this rep decided to add phone service to my account. Furthermore, the rep cancelled my online order and placed another one herself resulting in an additional $49.00 charge.
After being on the phone for over an hour, I was able to get the 149 phone installation fee and the 49 tv installation fees adjusted.
The problem is that, according to the website, I still have the phone service and I am being overbilled!!!!
Isn't it illegal to add services without the customer's permission?
I could cancel the service online, but that won't give me a refund for a service I did not request to begin with.
Help!!!
